If you are unable to solve an issue after checking our FormBox Troubleshooting Guide - please contact support@mayku.me with as much information as possible. Our team will review your request and contact you within 48 hours.
In order for Customer Support to help you effectively, you should include:
- Original purchase receipt
- A photo of the FormBox serial number
- A photo of a hand written note placed on the FormBox bed with today’s date and the FormBox serial number
- An address and contact number
- Confirmation you have the original box and packing materials
(It is recommended to keep both the purchase invoice and original packaging in a safe place, as they are required to make a warranty claim. If the original packaging is no longer available, replacement packaging can be purchased via support@mayku.me)
Every Mayku FormBox has been thoroughly tested and certified. Each machine comes with a two year warranty.
Mayku FormBox repairs within warranty are free. Standard RMA collection and return shipping to and from the original product delivery address is covered by the warranty.
We aim to repair or replace your faulty Mayku FormBox within 14 days of the machine arriving at one of our repair centers. Please note this does not include transit time.
